Clinical assessment understanding:
- We spend time at the first session, educating each patient about their underlying issue. X-rays (Radiography) are requested from some patients to help them understand our explanation of their condition. We cannot predict what views for X-ray (Radiography) are needed for a patient, without meeting the patient first hand. Kindly do not request for an X-ray over the phone. Be patient, dear patient!
- Always carry all your Past reports for your first appointment when you come to see us. It helps us understand you, your body and what your mind/ body has been through in the past. Your Past history is of immense importance to us.
- A lot of patients are not aware that Low Back issues can be the cause of your Knee pain or Shoulder blade pain. A Degenerative Knee issue/ Ligament injury at the Knee can be the result of a long standing Low Back issue. A lot of people only get Back stiffness, while some never get any Low Back symptoms at all! Ms Khambe had excruciating pain below her Shoulder blade. She was surprised to know that this pain was due to her Low Back. Upon assessment, we verified a L5 huge disc bulge, which when reversed caused an instant reduction in symptoms of the patient. At another time, Ms Khambe also came in for a subluxed distal phalanx which could not be judged on an X-ray. Clinical assessment helped us ascertain this subluxation and fix it, negating the use of a splint that the patient first came in with. Another happy patient!
Repeat understanding of your condition:
- Your complaints of pain are important to us and we sympathise with you for your pain – but remember that it’s good to have the pain that you are complaining about. Removing this pain symptom, will not relieve you of your underlying problem that resulted in this pain in the first place. Understanding what is causing your pain symptom OR what is the root cause of your pain symptom (or other complaints/ issues) is MOST important than just addressing the pain itself. Treatment of the underlying root cause, ALWAYS, ALWAYS, ALWAYS rids you of your Pain symptoms. We keep emphasizing this for you, till your perception changes. You need to change the way you think, so your body has a shot at recovery.
- Several patients only aim at Pain relief and rely on painkillers and anti-inflammatories. Relieving pain may seem like a good idea at first, but this pain returns when you stop taking your pain medications. Sometimes this pain disappears, only to return with more severity after a few months or years.
Sandra Dsouza had loud crepitus due to Patella Cartilage degeneration (knee cap bone). Any exercise suggested to her would cause uncomfortable grating crepitus (sounds) which were not re-assuring to the patient. “Cartilage shaving” (with consequent regrowth) exercises were given to patient for Homework. Patient showed absence of Patellar grating within a week of daily exercises.

- The body knows what it is doing and why it is complaining. It had fantastic
– Compensatory strategies,
– Muscle locking – unlocking strategies,
– Fascial orientation changing strategies and
– Biomechanical faults to overcome underlying compensatory strategies.
Changing these issues changes how the body performs and is ALWAYS the key to recovery in any Neuromusculoskeletal condition.
- Symptomatic relief is NEVER ENOUGH
Mr S. Subramaniam complained of Heel pain every morning. Little did he know that the problem did not really originate in his Heel, but his Back. Acute injuries and symptoms:
- Acute injuries caused due to external trauma (surgery, RTA, sports) need due assessment for proper recovery. These acute injuries sometime happen due to our own negligence and sometimes due to faults outside of our control.
- However, the majority of our population suffers injuries from internal misalignments (Biomechanical faults); which if understood well and corrected in time, can prevent acute injuries or even help them get better faster.
- Majority of our Footballers end up tearing their ACL during play time – due to a biomechanically unsound Knee joint which is already at risk. Such a knee cannot withstand the sudden acceleration-deceleration forces during Football games, due to its poor feedback from pre-existing Biomechanical faults at the hip, ankle or low back. Correcting these faults or compensatory muscle strategies from time to time can help a lot of Footballers from getting their ACL’s ruptured.

Recruitment concept:
- This is an old Muscle Physiology concept which has been put to good use by us to help patients. We focus on Muscle Control and Muscle recruitment in order to help Strengthen a muscle.
- At traditional Physiotherapy clinics, most patients are left with 10-15 exercises which they can do at home without supervision. There is however, no accountability for the muscles that actually help to bring about those exercise movements.
- The Compensatory strategies in the Human body are numerous and varied. Due to such Compensatory strategies, people end up using the wrong muscles to get the final exercise movement that has been suggested to them. It is important to ensure that the right muscles are being “activated” during any particular exercise movement – while the compensating muscle is made to shut off and focus on its own activity. This needs skill and focus on the part of the Physiotherapist as well as the patient.
- FACT: Most patients do not really focus on their exercises.

Vertigo and Dizziness Therapy (Vestibular Rehabilitation):
– Vestibular Therapy for Vertigo and Dizziness is a branch of Physiotherapy.
– We specialize in Vertigo of all Origins.
– Vertigo is the sensation of spinning or dizziness. People who experience vertigo may also experience nausea, vomiting, sweating or abnormal eye movement. More severe cases of Vertigo may include symptoms such as double vision, difficulty speaking, a change in alertness, arm/leg weakness or an inability to walk.
– We specialize in Vertigo and Dizziness treatment that have the following origins:
- Cervical Vertigo: Patients who have Cervical Spondylitis or Higher Cervical pain (C1-C2) tend to often experience a typical kind of Dizziness. This kind of Dizziness can be easily treated so as to prevent recurrent occurrence. Patients are explained about the source of Dizziness through Neck X-rays, and subsequent Physiotherapy treatment is provided to the patient. Unfortunately, patients with Cervical origin Vertigo are given Vertin, Stugeron and Stemetil (standard medications given for Vertigo) which do not help remove Cervical Vertigo symptoms. Patients continue to suffer without knowing the actual cause of their complaints. Cervical Vertigo is easily treatable and avoidable.
- BPPV: Some clients complain of Vertigo which originates from the Inner ear. When Calcium Carbonate crystals in the ear dislodge and block Semicircular canals in the inner ear, they lead to a very typical kind of Vertigo which a patient describes as ‘Room spinning’, ‘being thrown backwards or forwards’ or ‘body being flung’ feeling. A majority of BPPV can be completely resolved through proper diagnosis. There is no known treatment for Anterior Semicircular Canal BPPV, which is one of the toughest to treat. However, Dr Pinto has worked with some of the pioneers in this field in the USA, to help patients with Anterior canal BPPV. Such patients also temporarily benefit from the use of Vertin, Stemetil and Stugeron. Relocating the Calcium Carbonate crystals in the inner ear, often helps in completely resolving the different kinds of BPPV.
- Other Ear conditions: Certain other Ear conditions also cause Vertigo. Understanding the pathology of the disease often lets us help patients in resolving this kind of Vertigo completely. Sometimes, this kind of Vertigo tends to return, but we equip patients with the tools to help treat themselves at home should such a relapse occur.
- Stress related Vertigo: This Vertigo is often described as a ‘sudden quick push’ that the patient feels in the head. Its brief and quick, and tends to unnerve the patient. This type of Vertigo is observed particularly in women or patients who tend to worry a lot OR have excessive stress and poor sleep.

Temporomandibular joint Therapy (Jaw Joint Treatment):
– The treatment for the Jaw joint is also a branch of Physiotherapy.
– The Jaw joint is one of the most overused and under-treated joints in the human body. It is used 24/7 during talking, eating and breathing. However, patients tend to pay attention to this joint only when it restricts their mouth opening (eg.: after a dental procedure) OR when there is severe pain in the joint (eg.: while eating semi hard foods). We use advanced treatment techniques for treating Jaw Joint conditions that have the following symptoms:
- Jaw Clicking sounds,
- Jaw Grating/ Locking/ Popping sounds,

Maintenance Physiotherapy:
A lot of our clients, post their recovery come to us for “maintenance Physiotherapy” at least once a month. “Maintenance Physiotherapy” is a term coined by our clinic to assist patients who have degenerative conditions (Osteoarthritis), Chronic spinal disc derangements that need continued maintenance.
Certain Muscle-bone conditions tend to recur, while there are certain other conditions where patients are supposed to continue with a daily exercise regimen. However, several patients tend to stop focusing on their home exercise/care once their initial symptoms subside. For such patients, we recommend once/twice a month follow up sessions (after their initial discharge) in order to ensure that their chronic/degenerative condition is kept under a close watch. This helps the patient because the condition never increases in severity, even if the patient is irregular with exercises. Additionally, the patient does not return to us a year later stating that their previous complaints have returned.
Eg.: Osteoarthritis is a degenerative condition for which Medical texts say that there is no cure. But through Clinical research studies, we have shown that Osteoarthritic changes in the Knee (like Joint Space Width and Osteophytes) can be reversed through aggressive Physiotherapy treatment. Such patients, once discharged from our care are scheduled once a month (or once in 6 weeks) to ensure that the patient is compliant with the exercise program and there is no reversal of Osteoarthritic changes. In the above images - one can see the Patellar Joint Space Width change in an Osteoarthritic knee, after Physiotherapy treatment. What you may find interesting is, that these images were taken in unilateral standing (standing on one leg). The Joint Space Width did not collapse despite standing on one leg.

Patient Bajirao Bhosale comes to us once or twice a month from Pune to get treated for his Low Back, Neck and Shoulders.
